Transitions

Program Description:

The Transitions Program offers transitional supports for young adults with developmental disabilities, as they leave the school system and prepare for adult life in the community.  Planning and coordination is done in conjunction with the School System and is vitally important to a successful transition for a young adult.

Transitions may include:

  • Development of a person-directed plan that outlines the individual’s goals and how they might achieve them.
  • Post-secondary education options.
  • Support Services offered by Community Living Mattawa, such as Community Outreach and Supported Living.
  • Employment opportunities and Job Readiness training.
  • Volunteer and leisure connections.
