Foundations Transitions

Program Description:

The Foundations Program offers transitional supports for young adults with developmental disabilities, as they leave the school system and prepare for adult life in the community.  Planning and coordination is done in conjunction with the School System and is vitally important to a successful transition for a young adult.

Foundations Transitions may include:

  • Post-secondary education options.
  • Support Services offered by Community Living Mattawa, which include Community Access Supports and/or Residential Supports.
